Path by Apocalyptica at the moment is my favorite instrumental. What it takes to make a good/great instrumental is variation, If it's the same thing over and over it gets boring, If it's someone who suddenly pops into the song with a guitar solo or something it gets annoying. It can't be to long or else it becomes both annoying and boring.

My favorite instrumental is "Black Moon Overture" by Nokturnal Mortum. Kinda reminds me of something that came out of FF Tactics or something...still good though. Runs through a variety of moods and incorporates many instruments but all seem to fit in perfectly. It shows off the bands talent and songwriting ability. Good stuff.
For an instrumental to be good I think it has to take you somewhere...since it cant tell you something it has to move you with its melodies. And all good instrumentals do that.

My favorite instrumental is Sairin Kata Tsubasa no Tenshi ~Advent OneWinged Angel~, which is the extremely well known theme song of Sephiroth from Final Fantasy VII. This time, however, Uematsu's band The Black Mages gives the song an extreme heavy metal edge that blends seamlessly with the choir and orchestra and adds an incredibly dark and Gothic mood to the entire piece. The jagged guitars, chanting chorus, and heavy orchestra really drive the piece's overall feeling of death and destruction...this song kicks so much ass!!

I like Pink Floyd Interstellar Overdrive
I can't really define what it takes to make a great instrumental, I just like it.

...There is so much good instrumental music I don't even know where to begin, but since this is more targeted towards pop/rock/metal type of instrumentals I'll just go with some of the stuff off David Bowie's Low or Heroes, or maybe Melody Laughter by The Velvet Underground or Flying by the Beatles.
EDIT: And there is nothing in particular that makes an instrumental good or effective; it's all relative to the situation (the artist that is playing it, the tracks that come before and after it, how well it fits the album, etc) and that determines what the instrumentation should be like. It's very unspecific.
